ことにする — “I've decided to…”
States a decision the speaker has actively made
Dictionary form (or ない-form) + ことにする frames a decision the speaker is making themselves — “I'll make it so that I do X.” ことにしている (the て-いる form) extends this to a standing personal rule or habit: 毎朝走ることにしています (I've made it a habit to run every morning).
How to form it

Watch out
ことにする (I decide) and ことになる (it gets decided/turns out) look almost identical but hand agency to opposite people — mixing them up is one of the most common N3 mistakes, so always check who's making the call.
